Our adult day center supports older adults and any adult with medical concerns, in maintaining independence for living in their own homes and communities. We are the largest adult day center in Dane County and the only center in the county that offers a custom program for people with dementia, Alzheimer’s, and other memory loss conditions. We provide a more affordable alternative to nursing homes and most in-home health care services.

Nichole Kraemer

Program Director

Nichole brings compassion, understanding, and knowledge working with the aging population. She has extensive health care experience previously working as a Certified Nursing Assistant, Activities Director, and in Social Services. Nichole works closely with participants, families, caregivers, and community agencies to provide informative tours of the day center facility. She assists with the enrollment process, and develops individualized plan of care tailoring services and activities to the individual’s current level of needs, interests, and abilities.

Kristin Hutson

Community Support Specialist

Graduating with a degree in Event and Planning Management from MATC.  Kristin did her internship at Hyland Park Assistant Living in Sun Prairie, Wisconsin, which allowed her to discover her passion working with the geriatric population. Kristin plans, creates, and facilitates activities specific to people with dementia. “I cannot be more blessed to work with this population.
